COVID-19 Booking information
The AstraZeneca vaccination will be the only Covid 19 Vaccination available through our Practice.
All COVID-19 Vaccination Clinics will take place at either our Golden Grove or Fairview Green sites.
Information on start dates and when our appointment book will open will be made available as soon as we receive supplies of the vaccination
Vaccination Clinics will take place 5 days a week.

How Do I make a Booking
Prior to booking your appointment, please check your eligibility. You can check this by clicking on the link below:
We will only be vaccinating people eligible in Phase 1b.
Appointments will be made:
a. Online via HotDocs/Health Engine or
b. by calling our surgeries on: 8251 3885 (Golden Grove) or 7078 6188 (Fairview Green).
Two appointments 12 weeks apart will be required.
All new patients to our practice will require an initial consult with one of our GP’s before making a vaccination booking.
